The Challenge

One of the leading Disease Management Companies based in US offering disease management services for Center for Medicare & Medicaid and other commercial insurers was looking at ways to minimize the cost for patients of the healthcare insurer - federal and private organizations.

The requirement was of a complete design, development, testing, implementation, and maintenance of a web based application with centralized database and a user friendly front-end to assist nurses to provide disease care services to enrolled patients. Key challenges faced were:

Changing requirements. Unique idea (disease management) with complex involvement of multiple entities had frequent changes in the requirements. To allow easy accommodation of these changes the business logic was moved into the business logic layer from the user interface layer Accommodation of more users into the system than originally planned. This was also solved by moving more of the business logic, which was embedded in the User Interface layer into the middle business logic layer. This also allowed for inclusion of changing business logic.

Drop in response times during peak usage of the system. The database design was optimized by including more indexes and by incorporating redundancy of data in some of the key tables to gain performance, which resulted in faster response times.

The Solution

End-to-End software solution to manage the patient’s diseases were planned.

Integrated Claims Processing & Disease Management System to enable Primary stratification based on ICD-9, CPT 4 and NDC coding standards with more than 150 flags. Through Secondary stratification patient risks are identified. Object-Orientation used to encapsulate the technology differences in communication sub-systems and to provide a uniform view of data transport layer to the systems engineer.

Authentication & rules to access data, Mechanism to retrieve data and Handling multiple Databases was used. Better optimized high performance technique used at the data service end. Third party device integration to collect the data over the distributed internet was enabled. Ensured HIPAA compliant process to develop and manage data.

The Package

The whole package is designed for reusability of the functionality shared by many different applications. Work can be distributed from the desktop to more powerful network servers, which increases performance and scalability. It also enables the most intelligent implementation of a system within the larger context of existing systems and network topology. To create smaller more manageable projects, large complex projects can be divided into simpler component projects. Business logic can be changed without requiring changes to the client, dramatically reducing the time required to implement updates and simplifying change-management.

We at Comat have provided dedicated support and helped our client to showcase the application successfully during our client’s site visit by a leading insurance organization. The integration with claims processing curtailed semi automated process reduction in patient claim cost, helping the client in reducing the claim cost.

The application is developed using .Net, ASP 3.0, VB 6.0, COM, COM+, IIS, MS-SQL Server 2000.

The robust and scalable web based disease management system currently handles more than 20,000 patients across US.
